Joe Biden Let Hospitals Hide Their Prices for Four Years and RFK Just Made Them All Pay for It

Jun 13, 2026

The American hospital industry has spent decades hiding its prices from the patients paying them.

Now Trump's HHS is done asking nicely.

RFK and Dr. Oz just sent warning letters to 519 hospitals across the country – and the hospitals that ignore them are looking at $2 million in annual fines.

Hospitals Have Been Hiding Prices Since Before You Were Born

This isn't a new problem.

Trump started the fight in 2019, when he signed an executive order requiring hospitals to publicly post their actual prices.

The rule took effect January 1, 2021.

Hospitals immediately started ignoring it.

In the entire first year of enforcement, the Biden administration penalized exactly two hospitals – out of thousands caught non-compliant.

Two.

That's not enforcement.

That's a wink and a handshake.

The hospitals learned the lesson fast: the rules don't apply to them.

So they kept hiding prices.

They buried data in spreadsheets packed with billing codes that no patient could ever decode.

They posted "estimates" instead of real prices.

They made sure that when you walked into surgery, you had no idea what you were walking into financially – and you'd get a bill three months later that bore no relationship to anything you were told.

That system wasn't accidental.

It was engineered.

RFK and Oz Are Ending the Free Ride

The Trump administration started moving on this in February 2025, when Trump signed an executive order calling price hiding exactly what it is – "insufficient accountability" that drives up costs for every American.

Now the enforcement wave is here.

Since April, HHS has issued warnings or corrective action requests to 519 hospitals.

Texas leads the list with 42 citations.

California isn't far behind at 38.

Alaska is the only state in the country where every hospital passed.

The hospitals on the list had to receive a warning or submit a corrective action plan – and if they don't comply, they're facing annual fines up to $2 million.

An unnamed HHS official told the AP that more warnings are coming.

RFK put it plainly: "For years, hospitals kept Americans in the dark about the true cost of care. Families made medical decisions without knowing the price – and too often got hit with bills that drained their savings. That ends now."

Dr. Oz's message to the hospitals was even simpler: post your actual prices or face real consequences.

This Is How You Lower Healthcare Costs Without a Government Takeover

Here's what Democrats will never tell you.

The reason healthcare costs keep spiraling isn't that there's not enough government control.

It's that there's not enough transparency.

When hospitals hide their prices, they can charge wildly different amounts for the same procedure depending on who's asking – and nobody can shop, compare, or push back.

A blood test that costs $30 at one facility costs $300 at another.

A knee replacement that runs $15,000 at one hospital costs $70,000 at the next one six miles away.

Neither patient knew the price before they went in.

That's the game.

Trump has been fighting this game since his first term.

The rule he finalized in 2019 was eventually upheld 2–0 by the D.C. Circuit Court of Appeals.

The hospitals lost in court.

Then they found a different way to win: they waited out the Biden years while enforcement collapsed.

Now that strategy is running into RFK, Dr. Oz, and $2 million annual fines.

When patients can actually compare prices, hospitals have to compete.

When hospitals compete, prices drop.

That's not a theory – it's exactly what happened when the rule first took effect in 2021 and the small slice of hospitals that actually complied started losing patients to lower-cost competitors.

The American Hospital Association knows it too.

They've been fighting this rule since Trump signed the executive order in 2019 – lost in court 2–0, then spent the Biden years waiting out the clock while enforcement slept.

The clock just ran out.
