Texas Children's Hospital paid $10 million to settle fraud charges for billing Medicaid with fake diagnosis codes to drug children with puberty blockers.
Now the federal government has 224 more hospitals in its sights.
What the federal government just found in 10 years of hospital billing records is going to make you sick.
How the Scam Worked
The scheme was elegant in its ugliness.
Insurance companies generally won't pay for gender procedures on children.
So doctors needed another code – something boring, something that wouldn't raise flags.
They chose "Endocrine Disorder, Unspecified."
That code covers things like thyroid problems and adrenal gland dysfunction.
It has nothing to do with gender.
A federal study found that only 4.7 percent of patients diagnosed with the condition actually had it.

But someone figured out that if you put that code on a child's chart instead of "gender dysphoria," the insurance company pays for the puberty blockers without asking questions.
From 2015 to 2025, hospitals across America billed $50 million in puberty blockers for kids aged 9 to 17 under that single fake diagnosis code.
Nearly $7.8 million of that came straight out of Medicaid – taxpayer money meant for low-income children's healthcare.
Another $11 million was billed for kids aged 13 to 17 under a diagnosis of "precocious puberty."
Here's the problem: by medical definition, a 13-year-old cannot have precocious puberty.
Someone wrote that code down anyway and sent the bill to your insurance company.
What Vance Is Doing About It
Vice President Vance didn't release a statement and call it a day.
He sent a criminal referral letter directly to Attorney General Todd Blanche.
Vance wrote that providers who miscode treatment to get insurance coverage for gender procedures that insurers wouldn't otherwise cover "should go to prison."
That's not bureaucratic language.
That's a prosecutor's ultimatum delivered by the second most powerful man in America.
The new HHS report – titled Wolves in White Coats – identified over 225 hospitals and health systems with pediatric gender programs and flagged suspicious billing patterns across the list.

RFK Jr. sent a parallel referral to the HHS Inspector General, identifying four specific cohorts of anomalous claims.
One of those cohorts involves Planned Parenthood clinics – flagged for billing cross-sex hormones to minors in states where it was prohibited.
The referrals go to both the DOJ and the HHS Office of Inspector General for possible violations of the False Claims Act, the Food Drug and Cosmetic Act, and federal fraud and conspiracy laws.
The False Claims Act alone carries triple damages.
This Pattern Was Already Proven in Court
This isn't speculation.
Texas Children's Hospital – the largest children's hospital in America – paid $10 million in May 2026 to settle exactly these allegations.
The settlement, reached with DOJ and Texas Attorney General Ken Paxton, confirmed the hospital billed Medicaid using false diagnosis codes to cover gender procedures.
Five doctors were permanently stripped of their hospital privileges.
The hospital was forced to open the country's first-ever detransition clinic – free of charge for five years – to treat children harmed by the procedures.
Cleveland Clinic settled a similar case weeks later, paying $308,000 and committing $2 million toward care for detransitioners.
That's two major institutions already caught.
The financial motive was never hidden.
The HHS report documents what hospital administrators knew but wouldn't say publicly: pediatric medicine is a money-losing operation for most hospitals.
Gender clinics changed that math.
A child diagnosed with gender dysphoria becomes what the report calls a "captive patient" – requiring puberty blockers, then cross-sex hormones, then surgeries, then lifelong follow-up care, each step generating tens of thousands of dollars in revenue per child.
Convince a vulnerable kid they were born in the wrong body, and you've created a customer for life.
The doctors knew it.
The hospital administrators knew it.
And they coded the billing to make sure insurance paid for every step – whether the insurer agreed to cover it or not.
